Our registered nurse will assess your needs, set up a care plan in collaboration with you and your responsible party, and review it with you as needed. We are flexible to make care plan adjustments to exceed our client’s expectations by providing them with reliable care. Our care team will monitor and ensure that the care continually meets your needs.

The care we deliver is individualized, and tailored to your needs. We are here to accommodate your needs 24 hours, 7 days a week, and on holidays. We offer services in the range of an emergency 2-hour visit to around-the-clock care, giving our clients unlimited options.

Our rates are reasonable and negotiable according to the duration and needs of the client. We provide assistance with activities of daily living such as but not limited to; *Bathing *Medication Reminders *Light housekeeping and Laundry *Pick up Medication and Grocery Shopping *Supporting Nutritional needs through meal preparation *Assistance with Oral Hygiene *Hands-on assistance with Ambulation and Transfers *Accompany to Medical appointments

If you are receiving MEDICAID, MEDICARE, or SSI we can help you to apply for home health care services through CCSP or SOURCE programs.
